ATM Dispute Form - Use this form to dispute an ATM transaction that has posted to your account.

Automatic Transfer (External) - Use this form to transfer money between accounts with Cincinnati Federal and another financial institution on a regular basis.

Automatic Transfer (Savings) - Use this form to set up a recurring transfer between two accounts at Cincinnati Federal.

CF Account Closing Form - Use this form to close your account with another financial institution.

CF Automatic Payment Change Form - Use this form to change your automatic payments to debit your new Cincinnati Federal account.

CF Direct Deposit Form - Use this form to change your direct deposit to your new Cincinnati Federal account.

Change of Address - Use this form to notify us of an address change.

Consumer Debit Card Application - Fill out this form to apply for a debit or ATM card. Subject to approval.

Lost Passbook Affidavit - If you have lost your passbook this form needs to be signed and notarized by all owners of the account before a new passbook is issued.

Seasonal Address Form - Use this form if your mailing address changes seasonally. 

Sweep Protection - Use this form to sweep money from your savings to your checking in the event your account is overdrawn and avoid any fees!
